	<p>USA Grant Applications is looking for your Grant or Loan Success Stories!  If you have already received a Grant or Loan as a USA Grant Applications Client, we will give you $250 for your Testimonial just for completing a few easy steps outlined below!
    </p>
    <p>Here's what we need from you to send you $250!!!</p>
    <ol>
    	<li><strong>Essay:</strong> Write your success story in 500-1000 words or 1-2 pages typed on a sheet of paper. Be sure to include a paragraph that specifically states exactly what you received (Grant, Loan, etc.), how much you received (actual dollar amounts), the interest rate (if applicable for loans), and the agency and program (or private source) you received it from and any terms and conditions. Also, state very clearly how we helped you.</li>
    	<li><strong>Documentation:</strong> Include any documentation you may have to substantiate your success, such as a photo copy of the actual letter informing you of your grant/loan.</li>
    	<li><strong>Photos:</strong> Include a photo of yourself and any additional photos that help tell your story.  Make sure your photos, are in color and in focus.</li>
    	<li><strong>Video:</strong> Include a DVD, VHS, or Digital Video of you telling us about your success story.  Please include as many details as possible about the type of money received and the things you learned in the USA Grant Applications Program.</li>
